I acknowledge that I am registering my child to participate in the FMYBL basketball program. I understand that this program may involve physical activity and participation in games, drills, and other related activities.

I confirm that my child is physically fit and able to participate. I agree to notify the camp staff of any medical conditions or concerns prior to participation.

I hereby grant permission for FMYBL to use photographs or video taken during the program for promotional or educational purposes. I understand that no names will be used without additional consent.

I acknowledge that all fees are non-refundable and that it is my responsibility to ensure my child follows all rules and guidelines set by staff.
